Job description
Visiting Nurse Health System seeks a Care Manager RN for Community Care to assess, coordinate, and monitor services for elderly or disabled clients in Hall, Barrow, Forsyth, and North Gwinnett counties. The role includes home visits, developing care plans, and collaborating with physicians and social workers to ensure continuity of care.
The ideal candidate has an RN license, is CPR certified, and has 2 years of community health or geriatrics experience.
