Job description
Mosaic Health, LLC. seeks a House Calls Advanced Practice Provider to deliver in-home clinical management for adult patients in South Tampa and Pinellas.
You will collaborate with the Medical Director and leadership to ensure timely, patient-centered, evidence-based care. Qualified candidates hold a master's in nursing and ARNP licensure, with strong documentation skills and the ability to participate in after-hours telephonic support.
