Job description
Lucet is seeking a Provider to deliver in-home and community-based care across Polk County, FL. You will conduct in-home visits, manage chronic conditions, and document in the EMR while maintaining HIPAA compliance.
Must hold an active, unrestricted license as a Provider in the state and be certified as FNP/AGNP or PA. Willingness to meet extensive background checks and maintain reliable transportation is required.
