Job description
Optum Home & Community Care, part of the Optum family, is creating something new in health care. As a HouseCalls Advanced Practice Clinician (NP or PA), you will conduct in-home health assessments for Medicare Advantage and other plan members, focusing on education, gap closure, and collaboration with primary care providers.
This role offers flexible schedules (20–40 hours per week), no on-call, weekends or holidays in many markets, and a comprehensive benefits package.
