Job description
UnitedHealth Group seeks a Nurse Practitioner (or Physician Assistant) for the Optum Home & Community Care program. This in-home role involves comprehensive home assessments, care coordination, and management of acute and subacute conditions with an interdisciplinary team.
No nights, weekends, or holidays, with daily travel to patient homes within the assigned area. You will perform history reviews, med reconciliations, vital signs, and point-of-care testing, and you will counsel patients
