Job description
Independence Health System Inc. is seeking a Direct Care Partner to assist consumers with daily living activities in their homes, helping them maintain dignity, function and independence while staying safe and comfortable.
Responsibilities include following care plans, providing personal care, meal preparation, mobility support, light housekeeping, medication reminders, errands and companionship. You will document visits, report changes to supervision, attend trainings, and participate in LEAN
