Job description
ConcertoCare in Brunswick, GA is seeking a Board-Certified Physician Assistant to deliver in-home and telehealth services to patients with complex needs within a value-based care model.
You will collaborate with a multidisciplinary team, manage medications, address social determinants of health, and participate in care transitions with a strong emphasis on patient relationships. Shift typically 8:00 AM–5:00 PM, with travel as needed.
