Job description
The House of the Good Shepherd in Utica, NY is seeking a dedicated Case Planner to partner with families, implement family assessment service plans, and oversee in-home services. You will monitor safety, manage behavior training for families, and arrange supervised visits as needed.
You will also provide 24-hour crisis intervention, coordinate referrals to clinical, financial, and community resources, and accompany families to appointments while maintaining documentation per agency policy.
