Job description
Signify Health, part of CVS Health, is hiring Full Time Clinicians (NPs or PAs) to perform in-home health evaluations for members in their own homes and to bridge gaps in care. The role supports a national network of clinicians delivering personalized health assessments.
You will conduct visits about 45 minutes, review medications and history, perform physicals, and, if required by plans, run selected diagnostic tests. Travel within licensed states and occasional virtual visits are expected.
