Job description
Signify Health is hiring full-time Nurse Practitioners or Physician Assistants to perform in-home health evaluations across Bridgeport, CT and surrounding areas. Clinicians will work in patients' homes, detailing health histories, performing physicals, and coordinating care with health plans.
Role requires NP/PA certification, unrestricted licenses, and the ability to travel locally within licensed states. In-home visits may include virtual consultations and diagnostic services as needed.
