Job description
Signify Health, part of CVS Health, in Charlottesville, VA, seeks a Part-Time Clinician (NP or PA) to perform In-Home Health Evaluations, visiting members at their homes nationwide. Travel to local licensed states and occasional virtual visits are required, with a 29.5-hour workweek.
You will review medications and medical history, perform a physical evaluation, and deliver Diagnostic & Preventive Services as ordered by health plans, without prescribing or altering treatment plans.
