Job description
Signify Health, part of CVS Health, seeks a Full-Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations. You will engage with patients in their homes, conduct comprehensive assessments, and bridge gaps in care, traveling locally and within licensed states as needed.
40-hour weeks with a strong emphasis on patient education and safety. The role includes a 45-minute visit per patient, a workflow-focused practice, and optional virtual visits.
