Job description
Signify Health, part of CVS Health, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations for health plan members in their homes. Visits last about 45 minutes and include a meds/history review and physical exam using an iPad workflow.
