Job description
Signify Health, part of CVS Health, seeks a Full Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations across a defined area. You will connect with members in their homes, review medications and history, and conduct physical exams with optional diagnostic testing.
You will travel locally and nationally, coordinate with health plans, and use an iPad-based workflow. Multi-state licensure and board certification are required.
