Job description
Optum Home & Community Care, part of Optum, is expanding its HouseCalls program to deliver preventive in-home care to Medicare Advantage members. As a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will perform annual in-home health assessments and collaborate with PCPs to improve outcomes.
This role offers flexible scheduling (8am-8pm across 7 days), no on-call, weekends or holidays, and a $10,000 sign-on bonus for external candidates.
