Job description
Pathway Geriatrics, a nurse practitioner-owned medical practice headquartered in Redmond, WA, seeks an experienced Geriatric Nurse Practitioner to provide palliative primary care to homebound older adults in the Seattle Eastside region. The role is 100% field-based with weekly meetings at the Redmond office, typically 9:00 AM–5:00 PM, Monday–Friday.
You will manage a panel of about 200 patients, coordinating care across AFHs, private residences, and assisted living facilities.
