Job description
Strengthen Physical Therapy, Inc. is seeking an Experienced Physical Therapist to join our team for home-based patient care in Valley Stream, New York.
This full-time role requires a NY-licensed PT who can evaluate patients in their homes and develop individualized rehabilitation plans. You will work within a multidisciplinary team, document progress in electronic health records, educate patients and families on home exercise programs, and ensure compliance with state and federal standards.
