Job description
Matrix Medical Network seeks a Nurse Practitioner or Physician Assistant (PRN) to perform comprehensive in-home health assessments for health plan members. You’ll complete histories, physical exams, and risk assessments, identifying care gaps and social determinants that impact health.
You will document findings in EMR systems, educate patients and caregivers, and collaborate with physicians and health plans to improve outcomes.
