Job description
Signify Health is seeking a part-time In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for adults in their homes. In this role, you’ll conduct detailed histories and physicals, reconcile medications, assess social determinants of health, and identify gaps in care for vulnerable and chronically ill patients. You’ll document findings in an EHR, collaborate with remote clinical teams, and support value-based care programs that improve outcomes and reduce costs.
