Role Overview
This position involves conducting comprehensive health assessments in members' homes or care facilities, focusing on adult and geriatric populations. The provider reviews health history, medication adherence, social factors, and performs point-of-care testing to identify health risks and improve care quality.
Key Responsibilities
Perform in-depth health assessments, including medical history, diagnosis, treatment, and health education.
Order and administer diagnostic tests as needed.
Collaborate with physicians, case managers, social workers, family members, and caregivers for coordinated care.
Provide follow-up and referrals to appropriate healthcare services.
Document encounters accurately following protocols.
Work flexibly at various locations, including members' residences and care facilities.
Qualifications & Skills
Master's Degree or equivalent with NP licensure, including prescriptive authority.
Board certification in Adult, Gerontology, Family, or Emergency Care.
Minimum of one year NP experience, current BLS/ACLS/CPR certification.
Ability to travel frequently, including overnight stays, and possess a valid driver's license and insurance.
Strong computer skills and excellent communication abilities.
