Job description
Matrix Medical Network seeks experienced NP providers to perform comprehensive in-home health assessments across the United States. Visits can span up to one hour and focus on medical history, medications, and social determinants to close care gaps within a single visit.
Providers administer point-of-care tests, coordinate with PCPs and care teams, and deliver care in patients’ homes or SNFs. Strong clinical credentials and state licensure with prescriptive authority are required.
