Job description
Matrix Medical Network seeks a Nurse Practitioner to perform in-home comprehensive health assessments for adults and seniors. You will review medical history, provide diagnoses and education, and coordinate care with physicians and care teams, while documenting findings per Matrix protocols.
Responsibilities include ordering tests, following up with PCPs, and delivering care in members’ homes or skilled nursing facilities. A valid NP license and prescriptive authority are required.
