Job description
Matrix Medical Network is seeking nurse practitioners to perform in-home comprehensive health assessments, reviewing medical history, adherence, and risks to close care gaps. Assessments can last up to one hour and may occur in homes or Skilled Nursing Facilities.
You will work with physicians and care teams to educate patients and coordinate follow-up, with schedule flexibility including weekends. A master’s-level NP with state licensure and prescriptive authority is preferred.
