Job description
Matrix Medical Network is seeking a licensed Nurse Practitioner to perform in-home comprehensive health assessments. The role focuses on evaluating medical history, medications and functional risks during visits often lasting up to one hour in members’ homes or SNFs.
Clinicians collaborate with physicians and care teams, order tests, and provide education and follow-up as needed. Flexible scheduling supports weekdays, weekends and extended hours nationwide.
