Job description
Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations for members in their homes, bridging gaps in care.
The role includes a 40-hour work week, travel within licensed states, and virtual visits as needed. You will conduct evaluations, review medications, and support plan of care with a patient‑centered approach. Full benefits are included.
