Job description
Signify Health, part of CVS Health, is seeking a Full Time Clinician (NP or PA) to provide in‑home health evaluations. You will connect with members in their homes, review medications and medical history, perform a brief physical assessment, and coordinate with health plans for appropriate care.
The role includes travel within licensed states, virtual visits as needed, and a comprehensive benefits package for full-time employees. A current NP/PA license and board certification are required.
