Job description
CVS Health Corporation seeks a full-time Nurse Practitioner or Physician Assistant to perform in-home health evaluations for patients in the Seattle area. Clinicians will use an iPad workflow, conduct comprehensive assessments, and support care coordination in members' homes.
The role requires active, unrestricted licensure, board certification, and ideally multi-state licensure to enable cross-state visits. Travel to local homes is expected, with virtual visits as needed.
