Job description
Signify Health, part of CVS Health, is seeking a Part-time Clinician (NP or PA) to provide in-home health evaluations in the Greeley area. You will engage with members in their homes, perform medical history reviews, physical evaluations, and discuss care navigation with plans in place.
As part of a national clinician network, you’ll travel locally to member homes, conduct virtual visits when needed, and deliver Diagnostic & Preventive Services.
