Job description
Signify Health, part of CVS Health, is seeking a Part-time Clinician (NP or PA) to perform in-home health evaluations in Grand Junction and surrounding areas. The role involves 29.5 hours weekly, conducting 45-minute visits, reviewing medications and history, and performing physical assessments using an iPad with a streamlined workflow.
Travel locally as needed and possibly virtual visits. Board certification and active license are required; multi-state licenses are preferred.
