Job description
Signify Health, part of CVS Health, seeks a Full Time Clinician (NP or PA) to perform In-Home Health Evaluations for clients in Portland, ME.
You will visit patients at home, take medical history, perform evaluations, and coordinate care, with travel across licensed states and virtual visits as needed.
