Job description
Signify Health, part of CVS Health, seeks a Full-time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations. Clinicians visit members in their homes, perform medication and medical history reviews, and conduct physical evaluations, with scope to order tests as required by health plans.
The role includes travel within licensed states, virtual visits when needed, and delivering diagnostic and preventive services.
