Job description
Signify Health, part of CVS Health, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.
You’ll conduct comprehensive visits, review medications and medical history, perform physical evaluations, and collaborate with health plans to connect patients with the right care.
