Job description
Signify Health, part of CVS Health, seeks a Part Time Clinician (Nurse Practitioner or Physician Assistant) to perform In-Home Health Evaluations, visiting members in their homes to bridge gaps in care and deliver flexible, personalized assessments.
You will travel locally and within licensed states, conduct virtual visits as needed, use an iPad-driven workflow, and provide Diagnostic & Preventive Services. This part-time role includes select benefits and a supportive team environment.
