Job description
VCU Health at Home program seeks a Home Health Medic to perform comprehensive assessments, diagnostics, and treatments in patients' homes as part of the Home Hospital model. The role collaborates within an interprofessional team to optimize outcomes and prevent unnecessary hospital use.
Under Nurse Manager supervision, you will conduct daily in-person visits, provide virtual support, and coordinate with nursing and provider teams.
