Job description
Signify Health, part of CVS Health, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations for members in the Fort Collins area. Visits last about 45 minutes, including medication/history review and physical evaluation.
You may perform additional tests as ordered by the health plan and use an iPad to follow a simple clinical workflow. The role offers flexible local travel, occasional virtual visits, and eligibility for benefits.
