Job description
Signify Health, part of CVS Health, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations for plan members across the New York area, bridging gaps in care.
The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and, when ordered by the health plan, additional diagnostic tests. You will use an iPad with an intuitive workflow and may travel locally as needed.
