Job description
Signify Health, part of CVS Health, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations in patients' homes nationwide.
The visits last about 45 minutes, include medical history review and physical evaluation, with optional tests as ordered by health plans. Travel locally and within licensed states is required; virtual visits are possible.
