Job description
Signify Health, part of CVS Health, is seeking a Part-time Clinician (NP or PA) to perform in‑home health evaluations for members in their homes. You will review medication histories, perform physical assessments, and coordinate with health plans to bridge gaps in care.
You will travel locally to licensed states, conduct virtual visits as needed, and document results using a streamlined clinical workflow on an iPad; no routine prescriptions are required.
