Job description
Signify Health is seeking a Part Time Clinician (NP or PA) to provide in-home health evaluations, visiting members in the comfort of their homes and bridging gaps in care.
Visits last about 45 minutes and include a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests. You’ll use an iPad and a streamlined workflow for each evaluation.
