Job description
Signify Health is seeking a Part-Time Clinician (NP or PA) to perform in-home health evaluations, visiting members in their homes and bridging gaps in care. You’ll use an iPad with a clinical workflow, and visits include med history, physicals, and optional tests as directed by the health plan.
The role requires active NP/PA license(s), board certification, and ability to cover Bronx, NY and surrounding areas.
