Job description
CVS Health Corporation, in collaboration with Signify Health, is seeking a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations in Ontario, NY. Visits last about 45 minutes with a focus on medication review and health history, using a mobile workflow.
Travel within licensed states is expected, with virtual visits as needed. Active NP or PA license with board certification is required; multi-state licenses are preferred.
