Job description
Signify Health, part of CVS Health, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations. You’ll visit members in the comfort of their homes, review medications and medical history, perform a physical evaluation, and help bridge gaps in care.
This part-time role supports a national network of clinicians. You’ll travel locally within licensed states, use an iPad-based workflow, and may conduct virtual visits as needed.
