Job description
Signify Health is seeking per diem Nurse Practitioners or Physician Assistants to perform in-home health evaluations in the Bellingham area and surrounding regions. You will meet members at their homes, complete comprehensive assessments, and support care coordination.
Role requires NP/PA licensure, board certification, and ability to travel locally. Per diem schedule with base hourly pay and incentives.
