Job description
Signify Health, part of CVS Health, is hiring a Full Time Clinician (NP or PA) to provide in-home health evaluations. You will engage with members in their homes, perform clinical assessments, review medications and medical history, and coordinate with health plans to connect members with appropriate care.
The role requires an active, unrestricted licensure in Seattle King County, board certification, and flexibility to travel within licensed states.
