Job description
Signify Health, part of CVS Health, seeks a Full Time Clinician (NP/PA) to perform in-home health evaluations for health plan members. Visits last about 45 minutes and include medication/history review, physical examination, and optional diagnostic tests as ordered by the plan.
A required license and board certification are expected, with multi-state licenses preferred. Travel locally and virtual visits may be required.
