Job description
Matrix Medical Network is seeking a licensed Nurse Practitioner to perform Comprehensive Health Assessments in members' homes or Skilled Nursing Facilities across multiple states. You will review medical history, medications, and risk factors, and coordinate care with PCPs and case managers.
We offer flexible scheduling, full-time opportunities, mileage reimbursement, a 401(k), and ongoing training. Travel up to 80–90% is required, with overnight stays as needed.
