Job description
Matrix Medical Network is seeking a Nurse Practitioner to perform comprehensive health assessments in members' homes and SNFs. Each visit lasts up to an hour and includes medical history review, medication adherence, and risk assessment, with documentation per Matrix guidelines.
Responsibilities include coordinating with PCPs, case managers, and care teams; offering education and follow-up; and conducting point-of-care and lab testing as needed.
