Job description
Matrix Medical Network in Virginia seeks a Nurse Practitioner to perform Comprehensive Health Assessments in patients’ homes or facilities. You'll review health histories, medications, social factors, and risks; provide education, referrals, and follow-up; document per Matrix guidelines.
You will administer tests, collaborate with PCPs and care teams, and travel up to 80–90% across the state and possibly neighboring states.
