Job description
Signify Health, part of CVS Health, is seeking a Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations, visiting members in their own homes and bridging gaps in care.
The visit lasts about 45 minutes and includes a medication history review, physical evaluation, and optional diagnostic tests. You will use an iPad workflow and are not required to prescribe or alter treatments.
