Job description
Matrix Medical Network is seeking a NP/PA clinician to perform in-home comprehensive health assessments across the state. Each visit can last up to one hour and focuses on health history, medication adherence, social environment and risk factors to identify gaps in care.
The role supports coordination with PCPs, case managers and other professionals, and may involve travel to patients’ homes or Skilled Nursing Facilities to deliver care and education.
